Abstract

Diabetes mellitus (DM) is a chronic disease that requires continuous management. Effective discharge planning is key to improving the knowledge of DM patients to prevent complications and readmissions. In Indonesia, the prevalence of DM is projected to increase from 9.19% in 2020 to 16.09% in 2045, with an estimated 40.7 million cases. To analyze the implementation of discharge planning in improving the knowledge of diabetes mellitus patients based on current evidence-based practice. A literature review was conducted using a search of scientific articles from PubMed, Science Direct, and Indonesian health journals for the 2020-2025 period. Inclusion criteria included articles discussing discharge planning in DM patients, published within the last 5 years, and available in Indonesian or English. Structured discharge planning has been shown to improve DM patients' knowledge, reduce readmission rates, and enhance self-care adherence. Optimal implementation requires a multidisciplinary team approach with validated educational modules. Discharge planning is an important intervention in improving outcomes of DM patients by increasing knowledge and self-management skills.
